Thank you for your interest in Live Well Omaha's Sister Norita Cooney Lifetime Achievement Award. The award winner will be recognized at the Wellness Awards Gala, taking place during the evening of October 8, 2019, presented by WELLCOM and Live Well Omaha. Applications will be received until Friday, May 31, 2019.
Sister Norita Cooney, RSM, founded Our Healthy Community Partnership, now Live Well Omaha, in 1996 to address community health issues. Her spirit of philanthropy, business savvy, and compassion to always help others as if they were family has guided Live Well Omaha since its creation. This award honors a leader who has dedicated a lifetime to the achievement of healthier communities for all and made significant achievements in lasting policies, systems, environments and structures that support health where we live, learn, work, play, shop, worship, and connect.
To nominate someone or nominate yourself, please submit a written summary with a response for the following questions:
1) In 1-3 paragraphs, give an overview of the leader’s lifetime of work and passion for community health through their background, key achievements, and significant contributions. Did they influence or contribute to policies, systems, environments, or structures that promote health still today?

Applications will be accepted until Wednesday, May 31, 2019. Winners will be notified by Friday, June 29, 2019.
